Navigate to https://app.1inalltravel.com/forms

Click "Add New Form"

Enter form name

Add new field label

Select Input type for the field

Click "Add Field"

Add multiple fields same way

Delete fields if needed

Drag fields to rearrange order

Click "Copy" to copy the html and script of the form
